Instructions to USW Representatives
In order to provide you with the most competitive proposal, the Fund must receive certain information about your current plans. To facilitate this process, the Fund has developed a Request for Fund Quote/Information Form. To best serve your needs in bargaining, this information should be provided sixty (60) days prior to the expiration of the collective bargaining agreement.
- Request for Fund Quote – You complete this form which provides the Fund with information about the group, the benefit plans to be quoted and the timing for delivery of the proposal.
- Request for Information – You deliver this form to the employer to obtain information about the current plan(s) including rates, benefits, and covered employees. The completed information should be returned by you to the Fund office. Please make sure all requested information is complete. Missing information will delay the proposal process.
Once all information is received by the Fund, it is sent to the insurance company to develop premium rates for the requested benefit plans. A formal proposal is prepared by the Fund and two copies are sent to you or your designee. The proposal includes the rates, a summary of benefits, and a sample Participation Agreement.
The usual turnaround time for a Fund quote and proposal is about two (2) weeks from the date the request is received. Every effort will be made to accommodate quotes with shorter due dates.
